Her Song Volunteer Agreement *

Her Song is an at-will organization. This means that the organization may terminate the volunteer relationship at any time, for any reason, with or without cause or notice. Nothing in the Volunteer Application, or in any document or statement, written or oral, shall limit the right to terminate volunteer at-will. No officer, employee or representative of the organization is authorized to enter into an agreement - expressed or implied – with any volunteer.

Confidentiality Agreement *

I will not disclose any information I learn while in contact with the clients of Her Song. Such information includes, but is not limited to client’s names, personal information and/or physical descriptions of clients that could be used to identify
them, specific policies and procedures of Her Song, client's location, specific location, or geographic region of the Her Song home.

I will not to take photos and/or video of the property or clients. I will not post any content (photos, videos, and information) to social media platforms that pertains to the Her Song program or clients; however, I understand that I can send content to Her Song staff if it is a highlight that can be publicly celebrated. I understand that to disclose any of the above information to outside parties may result in me being forbidden to participate in the Her Song program, or in extreme cases, legal action being taken against me. If while on site you are ever in a position where you feel you must jeopardize the confidentiality of the Her Song program, please notify a staff member immediately. Such situations include your safety, client, or staff member’s safety, reports of current child or elder abuse.

Release, Covenant, Not to Sue and Hold Harmless Agreement *

THIS RELEASE, COVENANT NOT TO SUE AND HOLD HARMLESS AGREEMENT is

IN CONSIDERATION OF being permitted to work for, to volunteer with and/or to represent HER SONG, INC in any capacity, releaser, for them self, their spouse, legal representatives, heirs, and assigns, hereby releases, waives and discharges HER SONG, INC and its Board of Directors, employees, volunteers, representatives, and each of them, their legal representatives, heirs, and assigns, from all liability to the releaser, releaser’s spouse, legal representatives, heirs, and assigns, for any and all loss or damages, and any claim of damages resulting wherefrom, on account of injury to releaser’s person or property, even injury resulting in death of the releaser, even if caused by the negligence or negligent act or acts of the parties being released whether of omission or commission, or otherwise, while the releaser works for, volunteers with and/or represents HER SONG, INC in any capacity, at any facility or location. RELEASER EXPRESSLY agrees that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Florida, andthat if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal and effect.

Photo Release Form *

This photo release form is made by and between (“releasor”) and HER SONG, INC. (releasee) located at 10700 Beach Blvd #17807, Jacksonville, FL. 32245.

A. Releasor hereby consents and authorized releasee to use releasor’s likeness in a photo, video, or other digital media (the “photo”) in any and all of the releasee’s publications, including print or web-based publications.

B. Releasor irrevocably authorizes releasee to copy, edit, enhance, crop, or otherwise alter any photo for use in releasee’s publications. Releasor also waives rights for approval or inspection of any photos used in releasee’s publications.

C. Releasor understands and agrees that all photos are the property of releasee and will not be
returned to releasor.

D. Releasor acknowledges that this release has been entered into voluntarily, and the releasor is not entitled to any compensation or royalties with respect to releasee’s use of the photos.

E. Releasor agrees to release and forever discharge releasee and its affiliates, successors and assigns, officers, employees, representatives, partners, agents and anyone claiming through them, in their individual and/or corporate capacities from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, cause of action of any nature or kind, known or unknown, which releasor and any one claiming on behalf of releasor, may have or claim to have against releasee in connection with this release.

F. Releasor represents it has carefully read through fully understands all the provisions of this release and is freely, knowingly, and voluntarily entering into this release.
